They took another payment out and then told me I had to submit in writing. Gave my e-mai address to send the form. Then it had to come from another department, that was not in. Oh and by the way the form has to be in 30-days prior to service disconnection. I am guessing so they can attempt to withdraw another months monitoring service in a house we no longer live in. If I could do less than one star I would.
